Request to Join Georgia Tech's Active Directory

This form is for Georgia Tech employees only.
Please make the following decision once you have completed the appropriate forms and discussed your current environment with OIT. Before submitting the form electornically, please print a copy, sign the form, and retain it for your records.

The following options exist for integration of AD:

Option 1: Join as an Organizational Unit (OU) - I will completely manage and maintain my OU as described in the requirements documentation and as per the GTAD Forest Policy.

Option 2: Join as a Domain - I will completely manange and maintin my Domain environment as described in the requirements documentation and as per the GTAD Forest Policy.

Option 3: Do not participate with the GTAD Forest - Standalone AD Forest - I completely understand the ramifications of the decision to not participate with the GTAD Forest. I realize that OIT cannot provide any support for a separate AD infrastructure.
